Before a student-athlete is allowed to participate, Frisco ISD requires all
student-athletes to have a current physical exam on file as well as their
parents/guardians complete and submit the parental forms on the
RankOne Sports Portal for the school year.

It is strongly recommend that each student get their physical exam ASAP
to avoid the long lines in late July/early August.

Instructions for the physical exam and required parental forms:
Student-athlete must use the UIL Physical Form and have it completed
and signed by a legal physician.
